Though you don't like to admit it, you don't floss very often. Oh sure, you know it's important to remove the film of bacteria and food particles called plaque that builds up between and on your teeth. And you know you should do it every day.
It's just that, well… you're not very good at using dental floss.
While it's effective, dental floss takes some technique to hold it with your fingers and work it between your teeth. It can be hard for people to get the hang of it — and some aren't physically able or have obstacles like braces that make it harder.
There is a solution: an oral irrigator. Available for home use for decades, these devices deliver pulsating water at high pressure through a handheld device that looks like a power toothbrush. The water flows through a special tip to loosen and flush out plaque from between teeth.
You may have encountered oral irrigation during dental visits. They're a regular part of dental cleanings especially for treatment of periodontal (gum) disease. Because gum tissue weakened by disease may gradually separate from the teeth, large voids or gaps called periodontal pockets can form. These pockets can become further infected and accumulate plaque and calculus (hardened plaque deposits) that can also extend to the roots. Oral irrigation is a way to remove much of the plaque from these hard to reach places.
Oral irrigators have also proven effective for orthodontic patients whose brace hardware inhibits regular dental floss. A 2008 study, for example, found orthodontic patients were able to remove five times as much plaque with an oral irrigator as those who used only a manual toothbrush.

When he isn’t among the ten players vying for position on the basketball court, Cole Aldrich of the New York Knicks sometimes lets his dangle behind one ear. Mason Plumlee, who plays for the Brooklyn Nets, slips his snugly inside one of his socks during off times. Other players can be seen at timeouts gnawing on them nervously, or tucking them into spandex waistbands. But when the whistle blows, they go into the mouth before the players get on the court.
We’re talking about protective mouthguards, of course. According to a recent story in the New York Times, only a handful of basketball players wore them regularly a few years ago. Today, more and more are wearing them, both in practice and during games. What’s the reason for the change?
Partly, it’s the result of better design and improved construction; today’s mouthguards are more comfortable and better-fitting than ever. Part of it comes from mounting evidence that they work: Research studies have consistently demonstrated the benefits of wearing a mouthguard — not only to protect the teeth, but also to reduce the risk of concussion. And partly, it stems from the growing acceptance that safety is an important aspect of all athletic activity.
If the pros are doing it — shouldn’t you be too? While you may think that only contact sports (such as football) are risky, the fact is that “non-contact” sports like baseball and basketball account for the majority of dental injuries. In fact, the American Dental Association recommends you wear a mouthguard when you participate in activities like handball, soccer, surfing and weight lifting — as well as boxing, skydiving, martial arts and “extreme sports” — whatever that may mean to you.
What’s the best kind of mouthguard? It’s the one that you actually wear, of course! And the most comfortable and best-fitting mouthguard is the one that’s custom-made for you by your dentist. Sure, you can pick up an off-the-shelf mouthguard at a big box store; but it can’t offer you the same level of protection and comfort you’ll get from a mouthguard that is made from a model of your own teeth. Custom-made mouthguards from your dentist are reasonably priced — and by preventing dental trauma, they can save an untold amount of hurt… both to your body and to your pocketbook.
So take a tip from the pros: Get a custom-made mouthguard, and wear it every time you play. But when you’re done playing… how about putting it away in a case?

You may be among the one in three Americans who suffer from the pain of tooth sensitivity. Before attempting treatment, though, we must first identify the cause.
Your teeth are made of layers of different organic tissue. The pulp at the center of the tooth contains nerves that transmit pain or pressure sensation to the brain. The pulp is encased by dentin, a layer of tissue composed of tiny tubules that conduct temperature and pressure changes from outside the tooth to the pulp nerves. The hard outer enamel shell shields the dentin from over-stimulation from these sensations.
There are, however, some instances where the dentin may become exposed and cause sensitivity in the tooth. This can occur when the gum tissue recedes and the root of the tooth is exposed to the oral environment. If the root loses its surface coating (referred to as cementum, a cement-like outer layer around the root surfaces) because of over-aggressive brushing (too hard for too long) or advanced periodontal (gum) disease, sensitivity is often the result.
Another instance is enamel erosion. Although made of the hardest substance in the human body, enamel has one major enemy — acid. A high oral acid level brought on by over-consuming acidic foods and beverages or as a symptom of gastric reflux disease dissolves (de-mineralizes) the enamel’s mineral content. Brushing just after eating actually contributes to de-mineralization because the enamel is in a softer state. It requires forty-five minutes to an hour for your saliva to neutralize acid and restore minerals to the enamel — you may actually be brushing away enamel with this practice.
Once we know the underlying cause, we can use an appropriate method to reduce sensitivity. One way is to reduce nerve sensitivity in the dentin’s tubules or block them altogether. There are several chemical products for both home and dental office application that can reduce sensitivity and encourage enamel re-mineralization (as can the fluoride added to toothpaste). We can also strengthen enamel and provide a mechanical barrier to acid through concentrated fluoride in a varnish applied to tooth and root surfaces. And, life-like restorations like crowns or veneers not only improve the appearance of your teeth, they can also provide coverage for exposed dentin.
